No idea, never experienced it. Ensure you have the latest build installed for a start. Is it a flat or curved plane you are applying the decal to and how large?

I was looking through the forum trying to get help with this same issue. I am working with a file that has grown very large. In an attempt to condense it a bit, I ran the purge utility on it. I have done work in it since, but recently found when trying to place a decal that it crashes. I have tried doing an audit, but it doesn't find any problem with the file. I do not believe the problem is with the image I'm trying to attach, because I don't get that far. As soon as I click on "decal" on the modeling tool bar, it gives a message saying Revit has encountered a fatal error and has to shut down. I have reported to Autodesk several times, but have gotten no response so far.
