Revit 2018 (but had the problem in 2017 as well...) So I've had this problem for a while now. Can't put my thumb on it. But these two different voids just wont cut the object I've got.
TLDR version of my problem. Making a concrete panel, have 2 different types of reveals. A 2" large reveal, and a 3/4" deep Knock Out reveal. If they over lap, they wont cut. I can cut either one, just not both at the same time. After alot of googling people with the same problem have concluded it is because they are both 3/4" deep one decides it wont cut.
If i change the depth of one by as little as 1/64", they both will cut.
Hoping someone here has some insight. I'm stumped and would like a work around. I have many overlapping reveals on this job that are giving me a problem.
Ive stripped down an example as far as I can take it. It is attached. The image is what the panel should look like. I've had to cheat the reveals by 1/64" to make them work.

Blue, I wish I had Revit here to test this.... If the depth is important, what if the bginning of the void is different?

Try void profiles by sketch. Both of them.

WWhub & Pijpiwo,
I started to trouble shoot the problem first by making both by sketches (tracing the profile that was visible) the problem did not fix itself.
I changed the start/end points of the void sweeps.
It wasnt till i went back and modified the "face" side of my void making it deeper i noticed a small minor bust in the start point of one of my profiles. I'm assuming that because it did not start exactly at the origin that it must have been causing a problem.
So as usual, user error. It was just such a small minor problem that i could not see it. Whomever created the profile did a bad job.
So now it's working, by profile, with the fixed profile.

So what I've figured out - the angles between two faces will cause problems. If they are that close and do not clean up exactly, it will cause breaks. I've attached a graphic to explain it. This was happing on a different reveal that has now been fixed.
I've had to modify one of my 3/4" X 3/4" reveals to be slightly longer in the vertical direction due to the angle of the reavel on the front face.
