Elevation tags are system families. You can only choose from square or round and change their size.
Settings>View Tags>Elevation Tags
Custom ones are on the "wish list"

Try creating a new level...which is what I think you are looking for ..... new/annotation symbol...then choose "Level Head"...

you can get them to look like you want, but you will lose your parametrics with it. Just create an annotation symbol (generic) and then you make visibility parameters for each direction (up, down, left, right, or north, east, south, west, whatever). These would want to be instance parameters. Now, in your project, you place one of these symbols over the system family, make it how you want, and then turn off elevations in your view. If you want it to tell you what sheet # your view(s) are on, then you can place a text parameter in the center (or wherever you want it). The point being that you can do it, but it doesn't maintain the nice parametrics that the system family does. So I'd recommend just joining the rest of us in griping about Revit and looking forward to a new "fixed" version!
