I applied a Material to a loaded family of a door, this material is on other doors an looks correct. On this new door family its rotated 90 deg.

The material has a model hatch from a custom acad pat loaded into the material.

I edited the family, The hatch is fine in the family. 

What am I missing?

You don't say - are the doors where the material shows correctly the same family where it doesn't?  The way the material looks in the family may not mean anything.   The hatch pattern rotation is initially set in the material definition.   The material definition in the family may not be the same as the project.  'THE PROJECT RULES' .   Check the material definition in the family and verify it is the same..... NOT JUST NAMED THE SAME.

Good thought, yes the material in the family has a parameter assigned to it, in the project I set that parameter value to the material in the project. On futher inspection I found one other dooe family that is doing the same thing. Other doors in the project that have that material look correct. 

I'm continuing to wrestle with this one

OMG! the material is on both sides of the door extrusion, initially I rotated the hatch on one side, that did not change the other side! I rotated the hatch there and presto the mystery solved DUH!
