Can I do such a tag with that kind of leader? because in revit the arrows starts at the begining of the text no underline connected with arrow...

Spot elevation is closer but it is not an editable family  just shows the elevation

Sorry but no - there is no way to do this automatically. And, I doubt that AutoDESK will ever create this feature since there are very few users that do this. You may find some work arounds but I don't remember seeing any here.
oops - I just realized you said tag - not text. The leader for tags come from the center of geometry. You can try adding an invisible vertical line in your tag that is centered on the "underline" line. That vertical line wants to be at least double your normal tag height.
