I am trying to create a flat rectangular toposurface, very simple I know, but for some reason when I place the fourth point the surface splits into two parts.. Any ideas on how to correct this?

Are all of your points at the same elevation?

You have not told us enough about your process or the problem.

Are you sure the surface splits or is that a topo line?

All points are set at the default 0.0m. There shouldn't be any topo lines because it is a flat surface with all points set at 0.0m. Normally the fourth point would just extend the toposurface, but now it creates the square toposurface and created this random split

On the left is a new revit file, I have done the same process as the created toposurface on the right

Sorry but we don't think you did what you say.

Click on the edge of the topo, pick edit surface, then window it and post the image of the properties like I have.

That is strange.   How do you only have one decimal place on your elevation properties?  I always have three decimal places on metric or feet settings.  Could it be that a point is at a different non-rounded elevation?

The line does stay after the sketch is finished.

The reason there's only one decimal place is because the 'rounding' under project units is set to '1 decimal place'. I wasn't the one who originally created the file, maybe a point is at a different non-rounded elevation, how could I check for this?

Window select all your points and in the propertise, replace that 0.0 with 0.00 and see what happens.
