1. I have been searching for a while now for a pile cap or footing tag that would allow me to tag the elevation of the "TOP" of Pile Cap or Footing, but everyone I came across only allows you to tag the elevation of the "BOTTOM" of the Pile Cap or Footing.
Can anyone help?!?!?!?!
2. Is there a way to create or adjust a leader arrowhead to add a integral perpendicular to the arrowhead? See my picture I've attached.

1. Use Revit 2015, it know hows a built in "Elecation at Top".
2. No not by default. You could create a detail component with nested generic annotation for the arrow head etc along with parameter for your text/tag.

I have several shared parameters that report top of cap and then can also report things like top of pile and bottom of pile. I usually drive the other parameters with the "top of cap" parameter. Then, I have a tag that reads the shared parameters
I have an addin that I use to update the top of cap elevation. This makes the process really fast. The other parameters are then updated automatically. It works really well for me.
