Consider recording when a file is opened and by whom with space for optional comments.
We use a drafting view to leave instructions for next operator, eg please see design architect re model changes made on date

This is achievable in workset activated files. When making a save to central comments can be added and then reviewed by looking at the backups of the file. Alternatively, just make a sheet at the start of the set that has any design related notes on it...
