I am slightly confused on the different levels of BIM.  Sometimes I see 4 levels, other times I see 5.  My company and I have incorporated the 5 levels as such:

1. 2D Drawings/Non-geometric data, line work.

2. Generic elements shown in 3D

3. Specific elements/ Simple details

4. Shop drawing/fabrication detailed 3D models (MEP)

5. As built design.  All elements incorportated

I am seeing other takes on this.  Wanting a professional opinion as to what you or your company is doing.

Sorry, I've actually never heard of breaking down 'Levels of BIM' in that manor but having said that, what you have there makes good sense to me.

Here's the Wiki

http://en.wikipedia.org/wiki/Building_information_modeling

Here's Autodesks definition at a glance.

Building Information Modeling is the creation and use of coordinated, internally consistent, computable information about a building project in design and construction. In a BIM application the graphics are derived from the information and are not the original information itself like in general CAD applications.
