Just wondering if it would be possible to set up an annotation family that instead of having a leader arrow pointing to something you would position a number next to the item and that same numner would appear at the beginning of the text annotation in a key type format. This is sometimes neater than having arrow leaders flying all over the place. I know you could do it manually with text boxes but could it be set up automatically?

You cannot achieve what you are looking to do with leaders.  You could modify a line based text family that I created to somewhat get the look you are looking for.  I
