We have created several title blocks and everything works great….but when we insert the title blocks in all the new sheet parameter fields show up as “?”, is there a way to have a placeholder text like “XXXXX” or something similar to represent a field needing info?

Edit the label inside the titleblock family. There will be a space to enter a Sample Value for each parameter. You may need to overwrite parameter values when you reload, I'm not sure. It probably will not affect any titleblocks that have been placed on sheets, but it should work for any new instances.

You edit the value in the project. All you are changing in the family is the default value. Select a label, edit the label, then in the window showing the parameter(s) used change the 'Sample Value'. I can't post screenshots or be more specific at the moment, Revit isn't available just now.

I understand, and that is what we have been doing. But is there a way to define the default text other then "?" in the parameter so when a new sheet is created whomever is working on the sheet will see "XXXXX" the fields that need to be updated?

That's what you are changing when you change the Sample Value. Again, changing this and reloading the titleblock may not affect any existing instances already placed in you project, I have not tested it.
One other thing I just thought of, this may also not work for Revit project parameters (sheet name, project status, etc.). The default values for those parameters would be set in the project, not the family.
