Does anyone have experience in creating a grille that is mounted directly onto the side of the ductwork, any suggestions or ideas on how to do this?

You should be able to use a face based family to do this.

If I use a face based family to do this will the diffuser properly connect (associate the cfm) to the ductwork?

I do not use MEP so I can not answer your connection question. I have moved this thread to Systems Technical Support.
