This may be a silly question and looked around the forum a bit for an answer along with some google searches. 

I am using Revit 2011. The sheet that contains the title block is sheet A1-First Floor Plan. The default D Size Title Block for the sheet is missing/deleted by accident. How do I bring it back in if it is missing?

I know I can copy the first floor plan and create a new sheet and add the copied view into the new sheet, but I don't what to have to do that for numbering purposes and so on.

I assume you mean the views are still there.  Just find your title block in the annotation families.  Expand the category (+) - grab the titleblock and drag it onto your sheet.  Adjust ist location and all will be fine.
