Thanks

I made one in one and adobe using macros for text boxes, check boxes, pulldowns...makes it user friendly

I am working on getting this to be more interactive and less cumbersome for management by adding to our companies Share Point site...really recommend electronic tracking as it is way too much of a hassle to manage otherwise.

You will want the essential information:

Type?

Will it be scheduled? If so, what and how so you can make the correct shared parameters

Does it need to be parametric?

2D or 3D?

generic or manufacturer?

What version/release of Revit?

...things of that nature.

Wow - how big is your office?  We have +/- 25 users and an email & phone conversation is how we do it.

Hey Hub!!

By the time our implementation is 100% we are figuring roughly 150-200 users between all our divisions. High end would be about about 250, but I don't foresee that happening

 Corrected the count.

Brett -

You and I responded at the same time so my answer / question about size was not directed at you.  I could see using something formal with 50 + users but I think it is hard to manage this with just forms.

Your HTML would be good to look at.  <SMILE>
