I used to always use extrusions in Revit 2009 but I am finding the new methods for mass in Revit 2010 very difficult to get my head around.

 does anyone have an easy step by step method of creating a custom extruded mass?

Go here: http://designreform.net/

Go to the "Tutorials" and choose Revit then see the videos, maybe it help...
