I'm trying to figure out how to modify annotation tags and callout bubbles so they mask the background. The picture I attached is an elevation callout with the tile floor pattern showing through. Does anyone know how to modify the system family itself or load a user-made annotation symbol into the system family to fix this problem?

Hi Maranello, I see that the post is very old, and I have a problem very close to yours.
Have you solved it?

Expand family category in your project browser and pick the elevation family you want to modify (assuming that you have the family loaded already). Right click on the family name (highlighted, see pic1), pick edit and in the family editor, create a new type and draw masking/filled region. Load it to your project and apply modified family in the elevation type properties (pic2).
