In order for revisions to appear in you titleblock, you will need to add a "revision schedule" to the titleblock. Check the Revit help menu for more information.

Probably a little late to help the original poster, but thought I would add this in for future searchers: It is my understanding that the revision box on each sheet only displays revisions that occur on that sheet. The revision box in the title block of your Cover Sheet is probably set up the same as your other sheets. It's not tracking the revisions for the set, it's tracking the revisions for itself. My workaround to get it to display all revisions is to squiggle a tiny revision cloud where it can't be seen (like in a filled bar) onto the Cover Sheet for each revision. That way, all revisions appear. (Now back to my search for transferring all of the changes I just made in Revision #4 to Revision #5 because an outside engineer issued a change order with revisions that I forgot about.) Just FYI... revisions can be set to either be by project or by sheet. Settings>Revisions and then choose the method you want.

True, that's good to keep in mind. We have it set to "By Project" to keep the numbers consistent throughout, but the revisions still only physically show in the schedule on each sheet depending on whether a revision has occured on that sheet. This is something that was set-up by someone else, so I am not sure of the intricacies involved enough to know if that is Revit standard (how other users will be experiencing it) or not. Sometimes this trips me up in my proposed solutions, since I am by no means an expert. Thanks.
