I am making a template in which I need to schedule the revision history on the sheet.

I have succesfully made a titleblock that can do this when I am using the OTB templates, but when I use the same titleblock family in my own template the revision schedule doesn't populate.

Is there any settings I might not have turn on/off? I have made the template from scratch.

Did you create any revisions?

Manage tab / sheet issues-revisions

You then have to have a revision somewhere on the sheet or in a view on the sheet.

I did, but the revision schedule only populates in the OTB templates.

This sounds like you did not create a correct revision schedule on your titleblock.  Read this:

I have made my schedule like in this tutorial: http://revit.rayvinly.com/customize-revision-number/

However, it only populates when I use a OTB template, but not the template I have made from scratch.

Read the Help I posted.  What you posted has nothing to do with your titleblock.
